Your health information
The Notice explains permitted uses and disclosures of your health information, your privacy rights, the practice’s duties, and how to make a complaint. Acknowledging receipt of the Notice is separate from authorizing disclosure or consenting to treatment.
The approved Notice must reflect the practice’s actual operations, applicable protections for substance-use-disorder records, effective date, and privacy contact.
Privacy questions or complaints
You may raise a privacy concern with the practice or contact the U.S. Department of Health and Human Services Office for Civil Rights. You have the right to file a privacy complaint without retaliation.
