PRACTICE POLICIES
Clear expectations.
Respectful care.
Website summary for review. Your current signed practice agreement provides the terms applicable to your care.
Appointments & cancellations
Appointments are scheduled in advance, and length varies by service. Please arrive or log in on time. Provide at least 24 hours’ notice when cancelling or rescheduling. Late cancellations and missed visits may incur fees under the current fee schedule; insurance generally cannot be billed for these fees.
Fees & financial responsibility
Self-pay fees are $200 for an initial psychiatric evaluation and $100 for a follow-up visit, across all booking options.
Applicable copayments, coinsurance, deductibles, and self-pay charges are due at the time of service unless arranged otherwise. Ask about charges for forms, letters, or other additional services before requesting them. Notify the practice of insurance changes.
Communication & continuity
Use the practice’s designated patient portal for clinical communication. Responses may be delayed by clinical duties, weekends, or holidays. Keep recommended follow-up appointments and inform the practice of changes in contact information. When care needs change, referrals or transition planning may be necessary.
Emergencies
The practice does not provide emergency services. Call 911 for an emergency or call/text 988 for crisis support.
